When turning in the application please provide a DHS state physical forms from the pediatrician that was done in the last 6 months along with a certified copy of the child's birth certificate.
To get started, please fill out our Child Care Application.
Next, complete both of the following applications: Household Eligibility Application and Child and Adult Care Food Program.
If needed please fill out the Infant Formula/Food Waiver
If you qualify for subsidized card, please fill out our Child Care Assistance Application.
